University Points of Contact (UPOC) Enrollment Process

To enroll in the UPOC program, please submit the following information to the e-mail addresses at the bottom of this form:

• Current Rank (if active duty Commissioned Corps)
• Name
• Agency
• Work e-mail
• Work Address
• Work Phone
• Pharmacy school(s) for which to represent PHS pharmacy via the UPOC program

Paste and copy the following statement into the e-mail request:

By submitting this request, I understand I am to maintain a minimum of two annual contacts with the pharmacy school(s) listed above and that these contacts are required to be documented utilizing the following link: PharmPAC Activity Report Form

The UPOC program has a UPOC Workgroup assigned to contact UPOCs twice yearly to remind UPOCs to contact their schools. Expect to hear from this workgroup on a twice yearly basis.
