Electronic Official Personnel Folder (eOPF)

First and foremost, all active-duty officers should visit the Office of Commissioned Corps Operations (OCCO) web site and make certain that their Official Personnel Folder (OPF) has been scanned and indexed. If you do not know your password, phone the Help Desk at 301-594-0961. Those officers who cannot access their OPF online, should contact OCCO at phone number 301-594-3470 or by e-mail at opffix@dcp.gov.

Officers who can access their OPF online should review the online OPF before faxing in material. This should be done in an effort to avoid submitting duplicates. Material must be faxed to either of the following fax numbers: 301-480-1436 or 301-480-1407.

Please note the following guidelines:

  • On the upper right corner of all submitted sheets, include your name, Public Health Service (PHS) serial number, and category. For multiple page documents indicate page sequence (e.g., page 1 of 3, page 2 of 3, etc.).
  • If you include a cover sheet to your curriculum vitae (CV), the cover sheet must be numbered page 1 of the CV.
  • Continuing education documents and support documents should be grouped by year.
  • Licenses/registrations/certifications should be faxed to the Licensure Technician at fax number 240-453-6142.
  • Documents initiated by OCCO (e.g., personnel orders, Commissioned Officers' Effectiveness Reports, PHS awards, etc.) are automatically placed into the electronic OPF. These documents are not accepted directly from the officer.
