PharmPAC Awards

General Information:

The PharmPAC sponsors annual honor awards to recognize those in our profession who stand out as leaders in our category:

  1. ALLEN J. BRANDS PHS CLINICAL PHARMACIST OF THE YEAR AWARD
  2. This award recognizes the achievements of pharmacists in the PHS that provide traditional pharmaceutical services, with primary emphasis on activities accomplished within the past 12-18 months.

  3. MARY LOUISE ANDERSEN LEADERSHIP AWARD
  4. This award recognizes the achievements of pharmacists in the PHS in non-traditional or administrative roles, with primary emphasis on activities accomplished within the past 12-18 months.

  5. GEORGE F. ARCHAMBAULT PHS CAREER ACHIEVEMENT AWARD IN PHARMACY
  6. This award recognizes a senior level pharmacist (civil service or commissioned corps), serving under the authority of the USPHS, for outstanding career achievements and/or outstanding contributions to the pharmacy profession during their USPHS career.

  7. U.S. PUBLIC HEALTH SERVICE AWARD FOR MANAGERIAL EXCELLENCE IMPROVING PHARMACY SERVICE OR PUBLIC HEALTH
  8. This award recognizes a U.S. Public Health Service pharmacist (O-5/GS-14 or lower grade) who has improved pharmacy service or the public's health through managerial excellence. Primary emphasis is focused on activities with the past 24 months that clearly describe the impact of the accomplishment on the population served.  The accomplishment is beyond the normal expectations for an individual in that particular position.

  9. RADM JOHN BABB PHARMACIST RESPONDER OF THE YEAR AWARD
  10. This award recognizes a pharmacist (commissioned corps, civil service or temporary federalized responder) that has provided outstanding emergency preparedness, disaster response, and contributions to national or international public health threats within the past 12 months.

    Form and Instructions

Eligibility:

All U.S. Public Health Service pharmacists, both civil service and commissioned corps personnel are eligible for these awards. In addition, the temporary federalized responders are eligible for the Pharmacist Responder of the Year Award. Specific award criteria are described in each respective award abstract. Commissioned corps nominees need not meet Basic Level of Force Readiness to be considered eligible for any of the afore mentioned PharmPAC honor awards; however, it should be noted that consideration will be given to those officers in compliance. Basic Force Readiness standards can be found in the Manual Circular – Commissioned Corps Personnel PHS NO 377 – 07/02/2004.

Nominations:

There shall be no restrictions upon the number of nominations that an individual or an Operating Division/Agency, program or facility can submit; however, discretion and judgment should be used. Nominations may originate at any level within or outside the Service. Self-nominations will be accepted for all PharmPAC Awards with the exception of the (1) George F. Archambault PHS Career Achievement Award in Pharmacy and (2) USPHS Award for Managerial Excellence Improving Pharmacy Service or Public Health. Agency approval is not required, but the cover sheet must include endorsement by the nominee's immediate supervisor. Nominations should be for activities performed within the past 12 - 18 months or unless otherwise stated within the individual award criteria. Nominations received by the deadline will be reviewed by the USPHS Pharm PAC Award Committee and evaluated based on the accomplishment achieved and forwarded to the PharmPAC for final approval. All nominations must be accompanied by a completed cover sheet (attached to each award abstract). Cover sheets should be filled out according to the following:

GRADE/RANK, NAME, TITLE, ADDRESS.... Self-explanatory

PROPOSED CITATION.... Not to exceed 25 words

RECOMMENDATIONS.... Name(s), address (es) and phone numbers(s) of person(s) nominating the individual for the award must appear.

IMMEDIATE SUPERVISOR.... Signature of the nominee's immediate supervisor is not meant to be an expression of the supervisor's opinion of the merit of the nomination, but only an acknowledgement that the basic facts, as presented in the nomination, are correct. Additional comments from the immediate supervisor may be included. However, these comments should not be a continuation of the citation, if the supervisor is the individual recommending the nominee.

If a Commissioned Corps officer is the recipient of the Pharmacist Responder of the Year Award, their name will be forwarded to Office of Force Readiness and Deployment (OFRD) to be part of their nominees for the Commissioned Corps Responder of the Year Award. In the event that the PharmPAC selects a civil servant or a temporary federalized responder as the recipient of the award, then the Commissioned Corps officer that received the highest ranking from the PharmPAC will be forwarded to OFRD.

Justification:

  1. The justification statements are to be made on 8 - ½ x 11-inch paper, not to exceed 2 pages (single side with no smaller than font size 12) in length.

    (NOTE: If the justification exceeds 2 pages, the nomination will not be considered.)

  2. The primary emphasis will be on activities accomplished within the past 12 - 18 months or unless otherwise stated within the individual award criteria, and by personal example of activities that have evidenced dedication to the principles of the PHS mission.

    "The mission of the Public Health Service (PHS) Commissioned Corps is to provide highly-trained and mobile health professionals who carry out programs to promote the health of the Nation, understand and prevent disease and injury, assure safe and effective drugs and medical devices, deliver health services to Federal beneficiaries, and furnish health expertise in time of war or other national or international emergencies. As one of the seven Uniformed Services of the United States, the PHS Commissioned Corps is a specialized career system designed to attract, develop, and retain health professionals who may be assigned to Federal, State or local agencies or international organizations to accomplish its mission.”

    ******Activities Prior to 18 months will not be considered when evaluating the nominations (exception: George F. Archambault PHS Career Achievement Award in Pharmacy).

  3. The justification will speak specifically to the criteria (listed below) and will contain sufficient background information to enable a judgment to be made about difficulties overcome, effort expanded, dedication, demonstrated adherence to principles and the pursuit of excellence by the nominee.

  4. No other information is to be submitted on the individual nominated for the award. Examples of materials NOT requested are OISs, curriculum vitae (CVs), COERs, etc.
